Health First Family Care Center provides primary health care services through an intervention, prevention, and education model following that of a Health Home and Community Health Center. Primary care services are provided by a full range of health professionals with the core staff of Health First consisting of  M.D.s, Nurse Practitioners, Behavioral Health specialists Case Workers, part- time Nutritionist, LPNs,  RNs, Medical assistants , and several part-time physicians under contract for special services in the clinic, including prenatal. All Health First staff are licensed in New Hampshire in their respective fields.

In-office clinical and preventive services:

Within the office, Health First uses a preventive, client-centered model utilizing an assessment, health screening, and preventative service approach to the family practice of medicine. Clients entering the practice are assessed for health risks and acute problems, and a care plan is made jointly with the client for the delivery of health, social, nutritional and other supportive services. The clinical staff is organized in service delivery teams to assure continuity of care and ongoing case coordination.  We strongly believe in the team interagency service delivery network; a comprehensive, programmatic and problem solving model.
